However, Nishikori was recently exposed by the Japanese media outlet "Weekly Bunshun," revealing that he had engaged in an extramarital affair with UNIQLO model Azuki Oguchi (大口あづ記) before his wedding, and they continue to maintain their relationship. Reports indicate that he confessed to Oguchi that he could not marry her, stating, "Although I have no feelings for my wife, the children are very important." Despite this, the two have been seen dating and traveling together, described as "an open secret in the tennis world."

Reports indicate that in 2023, Oguchi began dating another man, but he discovered the affair and confronted Nishikori with intimate photos and chat logs. Ultimately, Nishikori's legal team intervened, not only forcing the other party to destroy the evidence but also signing a confidentiality agreement, after which Oguchi broke up with that boyfriend. Although they had a brief period of estrangement, the two reignited their romance in 2024.
